How can you destroy a stone by failing to facet it? They never get destroyed, only cracks and lose a slot. Please help, Thanks!

You have to fail every single facet. If I remember right, there's rocks that go up to just +4. So get a some of those and then "lose" the 75% facet 12 times in a row.
In other words it's 25% to the power of 12, which is roughly 1 in 17 million chance.
