XBOX Gamepad button occasionally stops working in dungeons

I have experienced an issue where sometimes inside dungeons the "B" button in an XBOX Gamepad stops working. I cannot cast any skill that uses that button in the combination (e.g. LB + B) or any item (e.g. RT + B). However, everything else is working normally and after I leave the dungeon the button works normally again. I also tried disconnecting the gamepad from the USB port and connecting again, but that doesn't solve the problem. I can use all corresponding skills in the keyboard, so it's not a missing resource for the skill or something similar.

This has happened across different dungeons and different characters. I am also sure the controller is 100% working fine, as I mentioned in every occurrence of this issue after I leave the dungeon it works fine again.
