I have experienced an issue where sometimes inside dungeons the "B" button in an XBOX Gamepad stops working. I cannot cast any skill that uses that button in the combination (e.g. LB + B) or any item (e.g. RT + B). However, everything else is working normally and after I leave the dungeon the button...
