You can always use the quest item directly from your inventory. There is a button in the bottom left corner of your inventory item. Click it and use the item from there.

Usually the costume is presented by an item in your inventory. You can click it any time to put it on.
If it doesn't helps with this quest, I think you can try to abandon it and start again.

This quest has an condition of your character being Lv50+. Did you reached that level?

You're correct. The mob can spawn in any of specified locations randomly. They don't spawn in all locations simultaneously.
